Official Results Submission Form...The following information is required to be included in the study. Please include as much information as you can. To ensure confidentiality, once this form is received, you will be allocated a number (ITR Number), which will be emailed, to you. It is suggested that you print a copy for your records and note this number on your copy.
Please quote this number if you submit further results for this particular dog. If you are not sure about any aspect of this form, please forward enquiries to thyroidregister@yahoo.com.au.
